We have an exciting opportunity for an experienced (who is willing to work towards their AMPH qualification) Level 2 or Level 3 Social Worker in our Trafford Older Adults Community Mental Health Team (CMHT).

This is a Secondary Mental Health Multi-Disciplinary Team (under GMMH Trust) working with Older Adults who have complex mental health needs, including Dementia and we are based at Crossgate House in Sale.

Duties will include the Care Coordination/Case Management of a caseload under the Care Programme Approach (CPA, social care commissioning and team duty work, The successful applicant would need to demonstrate at least 2 years post-qualifying experience and a working knowledge of The Care Act, Mental Capacity Act, Mental Health Act , CPA ,Safeguarding Adults etc.
