INTEGRATED URGENT CARE TEAM

SOCIAL WORKER

Permanent - 36 hours

Location: Crickets Lane Clinic, Ashton-under-Lyne or Tameside Integrated Care Foundation Trust, Fountain Street, Ashton under Lyne

Salary: Grade H. Scale 29 - £32,910 to Scale 34 - £37,890 with a profession bar at SCP 31 **

We are looking for enthusiastic and motivated practitioners to join our Integrated Urgent Care Team within the Community and Hospital. The Integrated Urgent Care Team provides short term interventions to maximise independence and responds to urgent/crises situations to enable people to continue living in their own home for as long as possible. The team responds to patients on hospital wards, A and E and MAU (Medical Assessment Unit) to prevent inappropriate admissions to hospital, as well as planning discharges from intermediate care facilities. The team is made up of staff from health and social care, working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible. The team provides support to Younger and Older Adults with varying degrees of disability, social complexities and vulnerability living in the community.

As a key member of this busy but supportive team you will be responsible for the planning and delivery of outcome based assessments, reassessments and support plans which are effective in promoting independence and meeting individuals outcomes and aspirations. You will also provide support to carers and families.

The Team offer a wide variety of personalised support, assisting people to promote their independence and supporting people with complex situations to ensure they have the maximum level of choice and control available to them. In some circumstances it will be necessary to ensure safe and effective support to protect individuals from harm or safeguard peoples human and civil rights.

You will receive regular supervision, support and training to ensure your continuous professional and personal development.

Experience of working with adults and previous employment in a similar position would be desirable.

It is essential that you hold a current valid driving licence and have access to a vehicle for work use.

You will need to hold a recognised Social work Qualification and be registered with Social Work England.
An enhanced DBS disclosure is required.
