Salary – £10,577 - £11,677 per annum (£19,171 - £21,166 per annum pro rata) plus 9% unsocial hours allowance

The shift patterns are as follows:
Week one Sun 3-10pm, Tue 2.30-10pm, Friday 3-10pm
Week two Wed 2.30-10pm, Fri 3-10pm, Sat 3-10pm

About us:
Ascot House provides a 24 hour Intermediate Care service. The Multi-disciplinary team provides physiotherapy, occupational therapy and social care support as required. We provide short term support with personal care & medication. Our focus is to support our patients to regain their confidence and independence to enable them to return home stronger.

Discharge to Assess provides 9 service users with a short term placement out of the acute setting completing a holistic assessment of their social care needs this may mean a return home with a package of care or a move in to 24 hour care.

About you:

You will enjoy taking personal responsibility to resolve issues and change what you do and how you do it to make a difference.

You will have good communication skills with both service users and other professionals.

About the role:
You will provide support to service users in Ascot House. You will provide support with personal care tasks, meals and any other support required. Ascot House’s remit is to promote independence and to reable the service user while providing person centred care at all times.

You will undertake all training courses as required.

You will be required to undertake all tasks conducive to the role.
