We require a Social Care Assessor in the Adults Community Social Work Team and the role will be borough wide. The function of the post holder will be to carry out Care Act assessments; this applies to all Trafford citizens, either in the community or in residential care.

The successful candidate will require an understanding of the assessment and review process/function, incorporating knowledge of care & support planning and its purpose. Detailed understanding of the National eligibility criteria under the Care Act 2014 is essential and an awareness of how the domains are applied to determine eligibility for services.

The role involves actively completing the statutory assessments of adults & older people. You will need the ability to undertake outcome focused assessments in sometimes difficult, unpredictable community and domestic environments. This is a task for assertive people with strong skills of engagement and empathy who have the insight to develop new ways of working whilst always ensuring the needs of the service user are of paramount importance.

Assessment skills, computer literacy and excellent communication skills are essential for this post. You should also have access to and be able to drive a car.

Practitioners are rigorously performance managed and this measurement system demands the ability to accurately maintain records in accordance with departmental procedures including Trafford’s adult safeguarding policy and the deprivation of liberty safeguards (DOLS) guidelines detailed in the Mental Capacity Act (2005).

You will involve working alongside transformational colleagues delivering a new initiative for Social Care.
