About you

You will be supporting the implementation of our Adult Social Care Vision, providing the vital link between the community and social work within a multi-disciplinary/agency context.

About the Role
An exciting opportunity has arisen to redesign the face of health and social care as we know it in Trafford. We are developing our skills and interventions to reflect modern day social work practice and recognise that strengths-based, preventative approaches are essential to embedding the true ethos of the Care Act (2014). In Trafford we have implemented a strengths-based approach to embrace this cultural shift and are looking for dynamic, innovative locality workers with a strong knowledge of their local community assets and links to lead the way.
Our approach takes support provision back to its roots and moves away from the all too familiar bureaucratic system led approaches. We firmly believe that creating a ‘new’ relationship with people ( who may need support), creates improved opportunities, moves away from ‘traditional’ offers and produces more person centered outcomes for the residents of Trafford.
