Our multi-agency services for Looked After Children are looking for a suitably skilled and qualified Social Worker as we seek to improve outcomes for children in care and children leaving care.

•You will undertake the administration and management of a caseload of Children in Care and Children Leaving Care
•You will develop and implement Care Plans for Children in Care and Pathway Plans for children who are leaving care
•You will complete all statutory duties relating to the provision of services to Children in Care and Children Leaving Care
•You will develop, co-ordinate, implement and monitor in partnership with other agencies appropriate plans that address all aspects of a child's life
•You will be required to hold a Degree in Social Work (or equivalent) and to have had previous experience of working with Children and Families
•You must have strong communication and assessment skills
•You will have an ability to forge effective working relationships across all agencies
•You will ensure there are timely transitions for YP from the Leaving Care Team to Adult Services due to mental health or social care needs
