EDGE OF CARE SUPPORT WORKER

Full Time, 36 Hours flexibly over 7 days

Location: St Lawrence Road Children's Home, Denton

Grade F: SCP 17 £23,836 – SCP 22 £26,317

We have a vacancy on our Edge of Care team; the Edge of Care Service has a specific focus on minimising the need for children to become looked after by supporting families to stay together where it is in the child’s best interests and wherever it is safe to do so. Edge of Care Service ensures a more effective response to support families when they are in crisis. The intention is to bring together a range of services and activity across Tameside with a clear focus on supporting families to stay together and minimising the need for children to become looked after.

The Edge of Care Service builds on a family’s strengths and coping strategies, and incorporates a strong degree of challenge for them to change. The service is delivered to families for a time limited period (to meets the child’s timescales) with a clear contract and set of agreed outcomes.

The service works to defuse immediate and precipitating crisis within the home that leads to the service referral to deliver the agreed outcomes. The team use a combination of assertive outreach and practical support to engage families and change problematic behaviour. A strengths based support plan is developed and agreed with the family and those services working with them, using a ‘Team Around the Family’ approach to monitor and review outcomes for the family.

Staff provides a rapid response to children and families in crisis, enabling them to address and overcome the difficulties that have led to the family being at risk of breakdown, and prevent further escalation. The Edge of Care staff support families in the home without having to remove the child from the environment. Staff works alongside parents during what they deem to be a ‘crisis’, they can mirror responses to the young person and will mentor by example how to defuse the situation.

The team work flexibly across 7 days, on a rota basis which incorporates evenings and weekends and also links closely with the Council’s Social Care emergency Out of Hours Services.
