Salary: Grade H. Scale 29 - £32,029 to Scale 34 - £36,876 with a profession bar at SCP 31 **
** This post is classified as being in a professional job category. As such the grade for the job has a professional grade development bar which employees can progress through subject to meeting the following criteria:
1. Relevant professional qualification for the job role at Level 6 or above, AND
2. Minimum of 2 years post qualification experience in the role, AND
3. Maintained membership of a regulated body where this is a requirement to practice in the role where applicable, AND
4. Professional standards practised to the highest expected level.

Role Summary:
An opportunity has arisen within the South Community Mental Health Team for a Social Worker / AMHP to join us in working to provide integrated health and social care to people of working age who have moderate to severe mental health problems across a wide range of presenting conditions. The post will be based with the South Community Mental Health Team
There is a strong commitment to promoting your professional development. You will be supported by clinical and management supervision and will be given the opportunity to develop and enhance clinical skills, interventions and case management through a personal development and training plan.

There is expectation that you will either work towards your Approved mental health profession qualification or if you are already an AMHP then you will participate in the AMHP Rota usually around 2-4 duties per month and will receive an attractive allowance for this.

Key Responsibilities of the Post:
As a Care Co-ordinator, you will be responsible for the co-ordination, commissioning and personal delivery of individual packages of care & Treatment for a caseload of service users within our secondary mental Health service

Key Requirements of the Person Specification:
Qualified Social Worker with appropriate experience
in a relevant area of mental health practice.
Current driving licence or supported driver with use of vehicle.
